SERVICE CONTRACT

Thank you for trusting Your Last Story–Obit Support Service to help create or share your loved one’s last story. Below is the Terms of Agreement Service Contract for Your Last Story–Obit Support Service.

IMPORTANT: Please READ the TERMS below:

SERVICE CONTRACT–

By approving a service estimate, beginning a project, conducting any business with, and/or purchasing any products/services from Your Last Story–Obit Support Service, you agree to the full extent of the terms of agreement below:

DEPOSIT–

1) Created or Edit-Revised LIFE-STORY or LIVING TRIBUTE: There is a non-refundable deposit of 50.00 to cover consultation and gathering Story Facts.  After the deposit is paid, we’ll schedule a 15-30 minute consultation to review your story-facts, and then we’ll start the first draft to create or edit-revise the story. You’ll receive a conference call to hear the reading of the first draft within one business day.  Based on your feedback, we’ll make the needed changes and build a final draft for your approval. (Note–Please review your story-draft carefully. Once you confirm the story is finalized, there is an reentry fee to reopen your story to make additional revisions.)  After the balance is paid, we’ll email your loved one’s final draft and Family Tribute (if one is submitted) through Google Doc format; however, the finalized story-draft can be submitted through Pages or Word formats, if requested. 

CONSULTATION–

2) Created and Edit-Revision Life-Story-Projects each receive a consultation to gather story-facts. A 30 minutes consultation for a created story-project and 15 minutes for an edit-revision story-project. During the consultation, if an edit-revision is needed for Family Tributes, those revisions must be addressed during the consultation time frame. Should you, the Client, require or request additional consultation time, there’s an additional fee for the extended time. However, Your Last Story–Obit Support Service will assist and guide you, the Client, to meet your needs, yet with this understanding. (Call for extended pricing.)

SERVICE BASICS & ADDITIONAL PRICING–

Included–

3) Created and Edit-Revision Life-Story-Projects each receive a one-page story-project with the basic one-page plan. The life-story-project is prepared using 8×11 size paper with 1-inch margins, 11 font size text, 14 font size title, and 1.15-inch line spacing. (Call for pricing.) 

Additional Fee–

Should you, the Client, require or request additional Life-Story pages beyond the one-page in the basic story-plan, there’s an additional fee for the Extended Life-Story page(s). (Call for extended-page pricing.)

Included–

Each created story-project and edit-revision story-project consists of a complimentary one-page  Family Tribute page for edit-revision (refer to story-project preparation previously mentioned). 

Additional Fee–

Should you, the Client, require or request additional FamilyTribute pages, there’s an additional fee for the Extended Family Tribute page(s). (Call for extended-page pricing.

Additional Fee–

Once the finalized life-story-project has been completed and submitted for approval and returned to you, the Client, and should you, the Client, require or request additional revisions, there will be an additional Edit-Revision Reentry Fee for each reentry requested to address those additional revisions. (Call for pricing.) 

Included–

Your Last Story–Obit life-story-project is written in the language of English. 

Additional Fee–

Should you, the Client, require or request another language, such as Spanish, there’s an additional fee for Language Translation (Call for pricing.)

Additional Fee–

Should you, the Client request or require a cancelation of the life-story project, there’s a 50.00 Cancelation Fee if the project is canceled after 24-hours of deposit, excluding weekends and national holidays. Your Last Story–Obit Support Service promptly begins crafting your life-story project upon receipt of the deposit, using the sample draft approved by you, the Client. Therefore, a cancelation fee is assessed to cover the cost of the time spent on crafting the story project. Thank you

PROJECT TIMELINE–

4) ANY estimate(s) provided by Your Last Story–Obit Support Service is exactly that, an estimate of time. This estimate is not a guarantee of actual-time, nor is it a “not-to-exceed” time. With the utmost integrity, YourLast Story–Obit Support Service will strive to meet the estimated time frame; however, if for some reason Your Last Story–Obit Support Service cannot meet the estimated time frame, prompt and timely communication will be made before the estimated deadline to inform you, the Client, that more time is needed than estimated to complete the life story-project.

5) Your Last Story–Obit Support Service works with several third-party resources that we trust, but we are NOT responsible for the Terms of Agreement of these third-party resource affiliates. Any costs incurred from utilizing these third-party resource services that you, the Client require or request, which may arise in dispute will NOT be at the expense of Your Last Story–Obit Support Service.

MATERIAL AUTHENTICATION & PROFESSIONAL INTEGRITY–

6) The Client agrees to indemnify and hold harmless Your Last Story–Obit Support Service against all claims, costs, and expenses, including attorney’s fees, due to materials included in the Story-Projects (at the request of the Client) for which no copyright permission or previous release was requested or uses which exceed the uses allowed according to permission or clearance.

7) Your Last Story–Obit Support Service accepts no responsibility for typographical errors, spelling mistakes, or incorrect information on any project committed to print or production, including any changes or amendments requested after the Client confirms the life-story-project is acceptable for final draft production. We do the very best we can to make each piece publish-ready. However, it is the sole responsibility of you, the Client to proofread every element. Although Your Last Story–Obit Support Service may provide edit-revision service, and every effort is exhausted to check for errors, you, the Client, is responsible for approving the final draft by proofreading your, the Client content. The content given to Your Last Story–Obit Support Service will be used “as provided” or “created with Client permission.”

PROJECT ADJUSTMENTS DURING PREPARATION–

8) Any needed or requested services added-on during the course of preparing the story-project and Family Tribute will be billed according to the itemized price list. However, if the services requested, exceed the itemized pricing list, a tailored pricing plan will be constructed by Your Last Story–Obit Support Service. This tailored pricing plan is founded on what Your Last Story–Obit Support Service deems fair and equitable according to the time that the service(s) takes to complete. Before the project is completed, Your Last Story–Obit Support Service will inform you, the Client, of the updated cost of service within 24 hours, upon hearing about the needed changes, which are not addressed within the price list. You, the Client, maintain the right to decline service. However, Your Last Story–Obit Support Service will only release and return the portion of the story-project that was initially requested and agreed upon, after the balance for that portion has been paid.

DISCOUNTS & HARDSHIP–

9) Your Last Story–Obit Support Service appreciates when you, the Client, recommends or promotes our services. In response to your thoughtful gesture, a discount is shared for client referrals who request and pay for a Your Last Story life-story-project. You, the Client, will have access to the discount at your next service; however, you may sign a transfer-waiver, which allows another client to use your, the Client’s awarded discount. (Call to request a transfer-waiver.) Hardship comes at the most unexpected times, and Your Last Story–Obit Support Service is empathetic with clients who experience this hardship during a time when all they want to do is share the last story that honors the life and love shared for their loved one. With this in mind, Your Last Story–Obit Support Service, maintains the discretion to respond to your, the Client’s need for hardship consideration on a case-by-case basis. This discount is entirely discretionary on Your Last Story–Obit Support Services part. You, the Client, may not insist or demand a hardship consideration as a mandatory expectation placed on Your Last Story–Obit Support Service.

CONFLICT RESOLUTION–

10) Should you, the Client, find Your Last Story–Obit Support Services unsatisfactory, please make every effort to communicate with Your Last Story–Obit Support Services to reach an amicable resolution. Please honor this professional protocol. Please refrain from posting negative comments on any social media platform without giving Your Last Story–Obit Support Service an opportunity to address your, the Client’s concern(s), or disappointment(s). Suppose an impasse occurs, and arbitration is needed. In that case, Your Last Story–Obit Support Service and you, the Client, will agree to select an unbiased third party to address the matter.
